    In-Home Counseling

    • Home-based counseling to help seniors cope with difficult life events, address concerns about health, make important life decisions and plan for the future

     

    Health and Wellness Evaluation

    • Evaluation of the impact of physical and cognitive status on daily functioning
    • Assessment of mental health concerns
    • Review of other areas of life that impact daily functioning
    • Review of strengths and existing supports
    • Recommendations to enhance independence, safety, and quality of life

     

    Benefits and Services Evaluation

    • Access benefits for which elders are eligible
    • Access services that can help seniors maintain independence
    • Health Insurance Counseling (SHINE counseling by certified SHINE counselor)

     

    Housing Stabilization

    • Access services that enable seniors to achieve sustainable living situations
    • Provide Advocacy
    • Review housing options with seniors
    • Support elders to make challenging decisions

     

    Financial Stabilization/Monthly Bill Paying Services

    • Engage clients in establishing a budget
    • Facilitate compliance with monthly budget including timely rent payment
    • Help clients negotiate with creditors
    • Access discount programs and/or shut off protection
    • Debt management/ consumer rights

     

    Care Coordination

    • Maintain contact with elders, families, informal supports, and care providers to facilitate collaboration and continuity of care and to problem-solve.
    • Help seniors prepare for medical appointments by reviewing concerns
    • Accompany seniors to key medical appointments as needed

     

    Caregiver Support

    • Maintain contact with elders’ families and other caregivers to ensure collaboration and continuity of care and to problem-solve.

    This is an ongoing program.
